Perhaps a more important issue is not if he is an officer, or a clerk, but :
1. What is the job description of the person,
2. Was he trained/instructed in his job functions, roles and duties, if
yes than was there a record of the same, f not then his superior is
responsible for this dereliction of duty
3. Did he do the job, as per his instructions/training
4. etc.
I think unless the whole chain is fixed to perform, we can keep playing the
blame game. No improvement/change will take place.
