There is no necessity of any format to be used , RTI Act is a very simple and citizen freindly act , Any citizen of this country can sought the information by writing on a plain paper also , no need of typing also ,In our country except metros  most of the information seekers are from rural places , Only the thing the application must contains address of the PIO , Address of the applicant  , Details of information sought and initial fee of Rs. 10/- and application must be signed and dated
